Animal Encounter Intake Form |
Arkansas law requires that animal bites from mammals be immediately reported to the Arkansas Department of Health.
Rabies is a disease caused by a virus transmitted via a bite or scratch from mammalian animals (i.e. dogs, cats, raccoons, skunks, bats, etc.). It is important to take steps to thoroughly investigate animal bites/scratches, or other potential exposures, so timely medical care can be provided to protect people from the disease.
Please, fill out the multi-page survey below. The first form will collect contact information and will be followed by additional forms to submit information about the exposure and the animal encounter. These forms should be completed in one sitting and should take about 10 minutes. Someone will investigate your case.
